How to download records from the Patient Portal in Athena

Log on to the Patient Portal (only available until November 10th, 2024)



Click My Health on the left side of the page






















On the My Health page, click the Health Records tab.


Choose a time frame

Specify the dates of service for the health information that you want to download.

  • Custom date range—Specify the From date and the To date to retrieve health information from the dates of service in that time range.

  • Single date—Specify the date for the information, for example, that date of your last physical exam.

  • All time—Retrieve your entire health record.



Select the Hospital Stay Documents types you would like to receive.

Choices include Consolidated Clinical Summary, Referral Notes and Discharge Summaries.  You must select at least one document type to proceed.




Choose how you’d like to get your data:


The Download tab is automatically selected.  To download your health record, click the Download button.  A .zip file containing your health record is downloaded to your computer.


Please note there are other methods listed to get data, such as Email and Direct Exchange.  Do not use these other methods. It is critical that you only select Download as your option. Troubleshooting: If downloading your portal information for the "All time" range results in an error, try selecting "custom date range" and download the data in one-year increments.


Once the files have been downloaded to your computer, please follow these instructions:


Once it is finished downloading it will show here.


Left mouse click the icon next to the file above that says “Show in Folder”. The file will have a name similar to “Osceola-Medical-Center.zip”.




Right mouse click the file and then Left Mouse click “Extract All”, then click Extract.







Please remember the location of these files if you need access to them at another time.




If you are not reading this until after the Athena Patient Portal has been shut down and replaced by Epic MyChart - Please contact the submit a records request form to our medical records office to get your labs and records previous to November 10th 2024 - it can take 2-4 weeks to get these records delivered.
