What does transitional care look like?

Most patients typically stay somewhere between 10 to 14 days. Some patients with complex needs stay longer.

After a stay in Transitional Care, over 70% of patients are discharged back to home; the rest move on to long-term care. If, after your stay in transitional care, you are unable to return home, we will help you and your family with a placement.
