When you have chronic health conditions like high blood pressure or diabetes, prescription drugs can be a big part of your life. At Osceola Medical Center, our Medication Therapy Management (MTM) program connects you and your provider with a specially trained pharmacist to make sure all medicines you take are necessary, safe and effective.

An MTM pharmacist will help make sure that:

  • You are taking the right amount of medicine.

  • Your medicines are not interacting with each other.

  • You are taking only the medicines you need.

  • Any vitamins, supplements and over-the-counter drugs you are taking are safe and effective.

  • You are spending no more than you need for your medications.

How does the program work?

  1. It starts with a short phone call or one-on-one visit to review your medicines:

    • Medications for chronic conditions like diabetes, high blood pressure and heart disease.

    • Vitamins and other supplements

    • Over-the-counter medicines like aspirin and Tylenol

  2. We’ll look for ways to reduce side effects, prevent harmful drug interactions and lower drug costs.

  3. You’ll receive an Action Plan based on your needs to help you get the most out of your medications.

  4. We’ll stay in touch to keep you on a healthy track.
