Diabetes Care Program

81.png

More than 29 million Americans have diabetes, a disease that affects the body's ability to use blood sugar for energy, according to recent statistics from the American Diabetes Association. More than twice that number - 86 million - have prediabetes, a precursor to diabetes.

Our nationally-recognized* DiabetesCare program provides high-quality education services to those with this disease, with self-management as an essential component. Our program teaches the self-care skills necessary for better management of diabetes.

In addition to providing you with the necessary skills to manage your diabetes, we provide a support network that assists you in keeping track of necessary appointments to further manage this disease and help prevent some of the acute and chronic complications that can occur if not adequately controlled.

We can help you with:

  • The diabetes disease process

  • Nutritional management

  • Physical activity

  • Medications

  • Preventing, detecting and treating complications

  • Goal setting and problem solving
