The purpose of Osceola Medical Center's COVID-19 Self-Checker is to help guide your medical care decisions. It is intended only for adults who are 18 years and older and currently located in OMC's service area of Minnesota or Wisconsin. This form is not intended for the diagnosis or treatment of disease or other conditions, including COVID-19. Communication via our website cannot replace the relationship you have with a physician or another healthcare practitioner.

This self checker is intended for patients of Osceola Medical Center in Minnesota and Wisconsin. If you do not currently live in Minnesota or Wisconsin, please visit Centers for Disease Control and Prevention for more information.

When to seek emergency medical attention

  • New confusion
  • Inability to wake or stay awake
  • Bluish lips or face
  • Slurred speech or difficulty speaking (new or worsening)
  • Seizures
  • Signs of low blood pressure (too weak to stand, light headed, feeling cold, pale clammy skin)

At-Home Antigen COVID-19 Test is Recommended
We are sorry you are not feeling well. Based on your answers from the COVID-19 symptom checker, you should be tested using an at-home antigen test.
To schedule your free antigen test at Osceola Medical Center from 1/25-1/28, please use the link below. Until you get your test and its results, we recommend you get plenty of rest and quarantine at home to prevent the spread of illness. If you develop any of the emergency medical symptoms mentioned above, please call 911 or visit your nearest Emergency Department for medical care.

Schedule to pick up your COVID-19 test just north of OMC in the Wild River Fitness parking lot. Please follow signs, wear your mask and stay in vehicle for drive through service.

Pre-Operative COVID-19 Screening Swab is Recommended
Due to limited testing capacity and supplies, we are ONLY offering COVID-19 Pre-Op testing for our established patients at OMC. If you are not currently established at OMC, please reach out to your local health department or another testing site (see below).

If you are an established patient, please call 715-294-5680 today to schedule a pre-operative COVID 19 screening swab at Osceola Medical Center. When you call please have the following information:
- Surgeon’s name
- Date of your surgery
- Surgery Center's name, location and fax number of where to send your test results

- Have been boosted
- Completed the primary series of Pfizer or Moderna vaccine within the last 6 months
- Completed the primary series of J&J vaccine within the last 2 months
You should:
- Wear a mask around others for 10 days.
- Test on day 5, if possible.
- If you develop symptoms get a test and stay home.
If You Were Exposed to Someone with COVID-19 and one of the following:
- Completed the primary series of Pfizer or Moderna vaccine over 6 months ago and are not boosted
- Completed the primary series of J&J over 2 months ago and are not boosted
- Are unvaccinated
You should:
- Stay home for 5 days. After that continue to wear a mask around others for 5 additional days.
- If you can’t quarantine you must wear a mask for 10 days.
- Test on day 5, if possible.
- If you develop symptoms get a test and stay home
